If an individual is eligible for Medicaid, once they are within 90 days of release, they will have the choice the receive an expansive set of services to facilitate their transition back to the community, provide them with resources to successfully reintegrate into society, and reduce recidivism.

Re-Entry Targeted Case Management includes the following components:

• Health assessment to identify health concerns and unmet health needs
• Care plan to address the person's whole health
• Coordination with care providers in the community
• Warm handoff to another case manager to ensure continuity of care

Every individual incarcerated at Skagit County Community Justice Center is offered the choice to participate in Medication-Assisted Treatment. This treatment program is designed to support those who struggle with opioid use disorder and/or alcohol use disorder.

Medications given for Opioid Use Disorder include buprenorphine, methadone, and Sublocade. Medications given for Alcohol Use Disorder include Vivitrol.

Upon release from the facility, individuals are provided sufficient medication in hand as a bridge until their appointment with a community provider.

Weekly one-hour wellness classes focus on self-esteem, self-care, gratitude, SMART goal-setting, grief and loss, and other topics. The aim of the program is to empower individuals in the jail to explore ways that they can improve themselves, believe in their self worth, and create a more meaningful life.
