The Population Health Trust is a community advisory committee to the Skagit County Board of Health. Bringing together a diverse group of community leaders and partners, the Trust works collaboratively to strengthen connections and turn strategy into action, supporting better health and well-being for everyone in Skagit County.
Our Work
The Population Health Trust (PHT or Trust) works to create a healthier, more equitable community by using data to better understand the health and wellness needs of Skagit County.
We bring together qualitative data, such as surveys, interviews, and focus groups, with quantitative data, including disease rates, demographic information, and other health indicators. Together, these sources provide a more complete picture of our community’s needs and help inform decisions about how to address them.
The impact of the Trust’s work is collective, strengthened through collaboration among agencies, organizations, and community partners. Learn more about our work and who's involved below.
Community Health Assessment (CHA)
Community Health Improvement Plan (CHIP)
A Community Health Improvement Plan (CHIP) generally follows a Community Health Assessment (CHA). A CHIP is a long-term, strategic plan that identifies actions to address the health issues and priorities identified in the CHA. Our most recent CHIP focuses on food security. As part of this work, we collected additional data on food insecurity to better understand the scope of the issue and how it impacts our community. The website brings together local data to help community members, organizations, and public and private decision-makers better understand the health and wellbeing of our community and make informed decisions that support a healthier Skagit County.

Our vision for health equity in Skagit County is that everyone in our community has a fair and just opportunity for healthy living. This requires removing barriers to health that arise from poverty and discrimination. We are especially committed to eliminating disparities in health for excluded or marginalized groups within our community.
